Residents in these areas include oil-field workers with irregular shifts and agricultural laborers who move between counties seasonally. Both groups exhibit elevated A1C levels yet cannot upload glucose readings to centralized platforms because cell coverage drops outside major corridors like I-35 and I-10. State data from the Texas Department of State Health Services indicate that 68% of adults with diagnosed diabetes in the 32 border counties have never used a mobile app to log medication adherence.
Hospitals in El Paso and Laredo report that patients travel an average of 87 miles for endocrinology follow-ups, yet lack the digital record-sharing infrastructure that could allow remote titration of insulin. The result is preventable hospitalizations that cost Texas Medicaid an estimated $214 million annually.
The grant program addresses this barrier by requiring applicants to demonstrate deployment of offline-capable devices that sync when connectivity returns. Projects must target at least three of the 38 counties where diabetes prevalence exceeds 15% and broadband availability falls below 60%. Funded organizations receive preference if they partner with rural electric cooperatives already expanding fiber along transmission lines.
Unlike applications from Oklahoma or New Mexico, Texas requires explicit mapping of oil-and-gas workforce schedules into the technical specifications so that shift workers can enter data during 2 a.m. breaks. Applicants must also document how tools will interface with existing Texas Medicaid managed-care portals rather than creating standalone databases.
Successful proposals will allocate at least 35% of budget to device distribution rather than software development, reflecting the primary constraint identified in the state’s 2022 Digital Opportunity Survey. Evaluation criteria further require quarterly uploads of de-identified usage logs to the Texas Health and Human Services Commission for statewide benchmarking.
