Additional tracked outcomes include reduction in emergency-department visits for ambulatory-care-sensitive conditions among households below 200 percent of the federal poverty level.

Implementation Approach in Texas
Nonprofit applicants must demonstrate existing 501(c)(3) status and prior contracts with at least two Texas regional trauma advisory councils. Applications require submission of county-level claims data from the Texas Health Care Information Collection showing baseline utilization gaps and a signed memorandum with one or more licensed Texas Medicaid managed-care organizations.
