Many lack continuous insurance coverage because Texas did not expand Medicaid, producing uninsured rates above 18 percent statewide and exceeding 25 percent in Starr and Hidalgo counties. Community health workers operating from county health departments and federally qualified health centers therefore handle initial screenings, chronic-disease follow-up, and enrollment assistance without adequate training pipelines.
The available funding targets organizations that maintain Texas Department of State Health Services CHW certification programs. Eligible applicants must demonstrate existing memoranda with at least three county health departments or rural health clinics and must document plans to place trainees in counties where the primary-care shortage exceeds 40 percent of the national average.
Who Should Apply in Texas
Texas applicants must hold current state CHW instructor certification or partner with a certified entity. Proposals require letters of support from county judges in target counties and evidence that at least 60 percent of training hours will occur in non-metropolitan statistical areas. Budgets may cover stipends for trainees, travel to remote sites, and data systems that track encounters in Spanish and English.
Unlike Colorado applications, Texas requires demonstration of extreme rural delivery capacity due to frontier county designation.
