These counties report diabetes prevalence 2.4 times the state average, yet only 12 percent of diagnostic imaging equipment is located outside metropolitan statistical areas. Urban centers such as Harris and Dallas counties contain higher shares of foreign-born residents, with 28 percent of Houston adults identifying as Hispanic and showing elevated rates of hypertension and certain cancers that require targeted early-detection protocols.

The funding opportunity directs resources toward culturally specific screening programs that operate in both dense urban zip codes and adjacent exurban counties. Eligible applicants must demonstrate existing referral pathways to Texas-licensed providers and capacity to deliver services in Spanish, Vietnamese, or Mandarin within 90 days of award.
