This barrier stems from the state's decision not to expand Medicaid and from the concentration of low-wage employment in energy extraction, agriculture, and logistics sectors that frequently do not offer employer-sponsored insurance.
Residents in the Rio Grande Valley and the Permian Basin counties experience the steepest gaps. In Hidalgo and Cameron counties, more than 30 percent of adults report delaying care because of cost or inability to locate Spanish-speaking enrollment assisters. Urban core areas such as Harris County also contain pockets of underinsured immigrant households whose employers fall below the threshold for mandated coverage.

The grant funds positions that accompany individuals through Marketplace enrollment, county indigent care programs, and specialty referral processes. Funded projects must report quarterly on reductions in missed appointments and emergency department utilization within the served zip codes.

Priority scoring favors organizations already embedded in colonias or energy workforce communities. Data showing at least 40 percent of participants have household incomes below 150 percent of the federal poverty level must be supplied from the prior program year.
How Texas Health Navigation Funding Addresses Gaps
Unlike New Mexico or Arkansas applications, Texas requires explicit documentation of service delivery inside frontier counties where population density falls below six persons per square mile and where no federally qualified health center operates within a 60-mile radius.
